Locum terms and conditions

1) All pharmacist registered with Swift Locum Solutions (acting as an Employment Agency) must hold the correct and up to date qualifications required to complete the assignments placed through Swift Locum Solutions.

2) Pharmacists should be registered with The Royal Pharmaceutical Society of Great Britain (RPSGB) and be a permanent resident of the
UK or hold a valid work permit.

3) Conduct of Locums should be in keeping with and in accordance with the RPSGB Code of Ethics and Locums should adhere to this
standard at all times.

4) Pharmacists must ensure that they bring and display their Certificate of Registration and ensure that it is displayed at all times during
the assignment. Clients will reserve the right to refuse entry to any locum pharmacist who attends an assignment without the certificate.

5) Pharmacists are considered self- employed and therefore will be responsible for the payment of their own Tax and National Insurance
Contributions.

5) Payment for the assignment will rest with the client and Swift Locum Solutions will not be liable for any payment for the assignment. This
is the responsibility of the external client for whom the pharmacist is undertaking the assignment.

6) The pharmacist must hold valid Professional Indemnity Insurance.

7) The pharmacist is expected to act and dress in a professional manner at all times whilst on assignment with the client. Furthermore, it is
agreed that the pharmacist will arrive at least five minutes before the commencement of the assignment and leave at the agreed times.

8) The pharmacist agrees that they will not engage in a manner or conduct or suffer any act that will be to the detriment of Swift Locum
Solutions or the client.

9) Whilst at the client’s location, the pharmacist is subject to the client’s safety and security policy and must observe the rules and
regulations set out by the client.

10) The pharmacist is not employed by Swift Locum Solutions and therefore has no authority to act for or on behalf of Swift Locum
Solutions.

11) At anytime before, during or up to six months after the latest introduction to the client they wish to employ the pharmacist direct or
through another employment business, the pharmacist acknowledges that Swift Locum Solutions will be entitled to charge a fee to the
client for the introduction to them. The Locum agrees to inform Swift Locum Solutions of the employment. Furthermore, the Locum
agrees that they will inform Swift Locum Solutions of any occasions where self booking or direct booking occurs so the correct fees can
be made to the client.

12) Swift Locum Solutions will make various assignments available to you. You are not bound to accept any assignment that has been
offered by Swift Locum Solutions but by accepting an assignment you are agreeing to the terms and a binding contractual agreement is
made between you and the client, which you must honour.

13) If for some reason you are unable to attend a booking, then you must arrange for an alternative Locum to fulfil your assignment. The
replacement Locum must be registered with\or register with Swift Locum Solutions and be accepted by us and the client and complete
your agreed assignment at the original time and pay rate offered. Swift Locum Solutions will endeavour to assist you to find an
alternative locum to fulfil your assignment, however, where this is not possible, additional payment may be involved for which you will be
liable and any losses incurred by Swift Locum Solutions.

14) In instances where the Client has reason to seek compensation if you fail to complete an agreed assignment, the responsibility for
meeting this assignment will rest with you, the locum, and not Swift Locum Solutions.

15) All information passed to you by Swift Locum Solutions regarding any client or assignment is strictly confidential and remains the
property of Swift Locum Solutions. You must not pass any information onto another locum or any third party or introduce a locum to the
client except through referral to Swift Locum Solutions. Failure to comply with the above could mean exclusion from Swift Locum
Solutions for future work, other locum agencies being inform and a formal complain to the RPSGB.

16) If at any time before or during an assignment you become aware of any reason why you may not be suitable for an assignment you
shall notify Swift Locum Solutions immediately. You must also inform Swift Locum Solutions of any RPSGB or Police investigations or
any disciplinary actions in progress which may affect your continuing membership of the RPSGB or which might affect your ability to
obtain Professional Indemnity Insurance.

17) The locum agrees to maintain an up to date record of Continuing Professional Development programme in line with RPSGB guidelines
and supply details to Swift Locum Solutions when requested.

18) No alteration to these terms should be valid unless the details of such alterations are agreed by Swift Locum Solutions and the Locum inwriting and a copy of the varied terms is given to the Locum stating the date on or after which such varied terms shall apply.

19) In accepting an assignment secured through Swift Locum Solutions you are in agreement with the terms and conditions outlined above
and agree to abide by them.

Swift Locum Solutions are to provide you a locum solutions service, that is to say we will act as an agency as defined the under the Employement Agency Act 1973

You authorise Swift Locum Solutions to seek work on your behalf.

You have informed Swift Locum Solution that you wish to seek employment as a locum pharmacist.
